JOB SUMMARY: Provide one-on-one instruction services for a developmentally disabled adult.

ESSENTIAL JOB FUNCTIONS:

  • Provides daily support and instruction as needed.
  • Assists individual with performing work tasks.
  • Provides instruction on new work tasks.  Tracks individual’s progress daily
  • Reports relevant information, progress and program recommendations to the Developmental Training Coordinator and Case Manager.
  • Works as part of a team with Developmental Training and Case Management Departments in implementing program needs.
  • Educate, promote and provide support to individuals in making healthy lifestyle                                         

      choices, as referenced in the DSP health and safety and health and nutrition      
      training curriculums and/or service plans.

  • Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
  • Documents and reports all behavioral concerns to the Case Manager at the time of the incidents.
  • Accompanies worker on community integrated training sessions.
  • Monitors individual’s movement through the building for safety purposes.
  • The employee may be asked to perform other duties as required by the employer’s business needs.

MARGINAL JOB FUNCTIONS:

  • Ensures safety of all consumers under his/her supervision
  • Attends staffings as requested

PHYSICAL AND VISUAL ACTIVITES:
Communicating, understanding spoken language, far and near visual acuity.
Lifting over 50 lbs.

QUALIFICATIONS:

  • High School diploma or GED equivalent required
  • Valid and acceptable driving record required
  • Meet requirements of the TABE reading and math surveys required
  • One year of related experience preferred
  • Good interpersonal skills
  • Must be able to lift and transfer adult(s) from wheelchair to toilet and back
