DEPARTMENT: Life Skills Education
REPORTS TO: Health and Wellness Coordinator
JOB SUMMARY: Facilitates the implementation of programs and initiatives with the goal of helping individuals with disabilities to acquire the knowledge and skills to obtain an optimum level of health and wellness.
ESSENTIAL JOB FUNCTIONS:
- Assists consumers with identifying and performing fitness activities of choice
- Implement educational programs for consumers on healthy lifestyles utilizing the UICRRTC curriculum as the core curriculum
- Coordinate access to community recreational facilities and resources for consumers during leisure times
- Introduce health promotion activities to individuals in their residential setting using Health Matters Toolkits and other resources
- Assist in implementation of consumer health and fitness objectives as written in Individual Service Plans (ISP)
- Attend consumer Team Meetings and provide input and feedback on individuals progress on health and fitness objectives
- Attend residential house meetings in order to gather input from residential staff and communicate information regarding Health Matters
- Act as a liaison between the Life Skills Education Heath Matters program and Residential Department
- Collect and enter data for assessments, baseline, progress notes, final evaluations, and statistical analysis of individual and program outcomes, as well as other required paperwork according to time frames
- Assist Health and Wellness Coordinator in preparing materials in the form of flyers, newsletters and fact sheets in order to disseminate information regarding the Health Matters program to agency and community stakeholders.
- Assist Health and Wellness Coordinator in implementing training of personnel in their roles and responsibilities associated with implementing health and wellness plans/goals
- Responds to medical, mental health, and behavioral emergencies per agency policies and procedures
- Reports all behavior or medical issues to the individual’s RQMRP, Nursing Department, RPM, or case manager as appropriate
- Ensures that DSP, CPR, CPI, First Aid, UIC Train the Trainer, and any other required trainings are completed and remain up to date
- The employee may be asked to perform other duties as required by the employer’s business needs
MARGINAL JOB FUNCTIONS:
- Keeps abreast of developments in the field by attending in-services, seminars and workshops.
- Serves as an active member of the Health Matters Committee
- Volunteers for a minimumof one agency special function per year
PHYSICAL & VISUAL REQUIREMENTS:
1. Ability to lift/push/pull up to 50 lbs.
2. Ability to see accurately both near and far
3. Ability to walk, bend, twist, kneel, and stoop
4. Ability to accurately manipulate small objects accurately using fine motor skills
5. Ability to drive to various locations as needed, adapting to various weather conditions, lifting over 50 lbs.
QUALIFICATIONS:
- One year of experience in health and wellness programs preferred
- Current DSP certification preferred
- Eligible for PACE driver certification preferred
- Meet criteria of the basic reading (TABE) and math surveys
- Excellent written and verbal communication skills required.
- Valid and acceptable driver’s license/record.
- Knowledge of Microsoft Office Suite (Word, Excel, Outlook) required
