REPORTS TO: Residential Director

JOB SUMMARY: Provides health care monitoring of all people we serve in CILAs.

ESSENTIAL JOB FUNCTIONS:

1. Responsible for the monitoring of all self-medication procedures.
2. Participates as a member for the Community Support Team by attending annual and/or special staffings based on needs.
3. Provide education regarding side effects of prescribed medication.
4. Meet with each client on at least a quarterly basis to monitor individuals health concerns or as needed basis.
5. Provide consultation on health care needs.
6. Consult with individual physicians as needed.
7. Conduct semi-annual Tardive Dyskinesia screening on all individuals receiving psychotropic medications.
8. Educate, promote and provide support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or service plans.
9. Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
10. Provide medication certification training on a monthly basis or as needed basis.
11. Completes Physical Status Review and Self Administration of Medication Report annually by due date.
12. Completes the Nursing Assessment annually or as needed.
13. Administer medication as required/needed.
14. Obtain prescribed medication from pharmacy and distribute to clients as needed.
15. Provides on call nursing services as needed – on-site/phone consultation.
16. The employee may be asked to perform other duties as required by the employer’s business needs.

MARGINAL JOB FUNCTIONS:

1. Provide individual training to Independent Living Counselors for client medications and health care.
2. Participate in the Developmental Disabilities Nursing Association Quarterly Meetings.
3. Ensures the safety of persons being served.

QUALIFICATIONS:

• Bachelor’s of Science in Nursing (BSN) preferred
• Nursing License required
• Valid and acceptable driving record
• Excellent interpersonal skills
• Excellent oral and written communications
• Knowledge of Microsoft Office
